The secret of Imu and One Piece's immortality

This is one of the biggest and most central mysteries in all of One Piece, and while the manga has not yet given a complete, official answer, it has provided massive clues.

The "secret" is not a single fact but a combination of established in-world mechanics and the high-probability theory that connects them.

Here is a breakdown of what we know for a fact, and the dominant theory that almost certainly explains Imu's immortality.

1. The Core Secret: The Perpetual Youth Surgery

The most widely accepted and heavily foreshadowed answer is that Imu's immortality was granted by the ultimate ability of the Ope Ope no Mi (the Op-Op Fruit), which is currently eaten by Trafalgar Law.

  • What is this power? The Ope Ope no Mi has a final, secret technique called the "Perpetual Youth Surgery" (also called the "Ageless Surgery").

  • What does it do? It grants another person eternal youth.

  • What is the cost? The user of the Ope Ope no Mi dies upon performing the surgery.

This "immortality" is not invincibility—the recipient can still be killed in battle—but it means they stop aging and will never die from natural causes.

2. What We Know (Facts from the Manga)

  1. Imu is Extremely Old: During the Reverie, Imu is seen speaking to the Five Elders. Later, when King Cobra confronts them, Imu speaks of Nefertari Lili, an Alabastan queen from 800 years ago during the Void Century, in a familiar way. This all but confirms that Imu was alive and present during the founding of the World Government 800 years ago.

  2. The World Government Knew This Secret: Donquixote Doflamingo, a former Celestial Dragon, was obsessed with the Ope Ope no Mi. He knew about its secret surgery and stated that, combined with the "national treasure of Mary Geoise," it would have allowed him to rule the world. This directly links the fruit's ultimate power to the secret heart of the World Government (Imu).

  3. The Five Elders are Also Immortal: Recent manga chapters (especially in the Egghead arc) have shown the Five Elders (Gorosei) taking massive, seemingly fatal damage, only to regenerate instantly. They also have demonic, awakened Zoan-like forms. This confirms that Imu is not the only "immortal" one at the top; the five highest-ranking Celestial Dragons also possess some form of eternal life and unnatural power.

3. The Dominant Theory (Connecting the Facts)

The secret you're asking about is the logical conclusion of these facts:

800 years ago, at the end of the Void Century, a previous user of the Ope Ope no Mi was forced (or convinced) to perform the Perpetual Youth Surgery on Imu.

This sacrifice granted Imu eternal life, allowing them to rule the world from the shadows for 800 years, erasing the "true history" and ensuring their reign never ends.

4. New Complications and Evolving Theories

While the Ope Ope no Mi theory is the foundation, recent events have added new layers:

  • How did all five Elders get it? Did six separate Ope Ope no Mi users sacrifice themselves? This seems unlikely. This has led to a theory that Imu, or some other power, shared their immortality with the Gorosei.

  • Is Imu even human? Imu's name, when reversed, is "Umi," the Japanese word for "sea." Given that Devil Fruits are "hated by the sea," a popular new theory is that Imu is not an immortal human but a primordial entity—perhaps the "Sea Devil" itself—which would be the natural enemy of Devil Fruit users and "Sun God" Nika.

  • Possession? In a recent flashback to God Valley, Imu was shown to possess the body of Saint Saturn from a distance. This introduces a new possibility: Imu's "immortality" might be a consciousness that can move between bodies, making the host body (which may or may not have had the Youth Surgery) a simple vessel.

In summary: The "secret" is that Imu has ruled for 800 years. The most likely method is the Perpetual Youth Surgery from the Ope Ope no Mi, but new clues suggest Imu's nature and powers may be even more complex, possibly tied to a primordial sea entity or the ability to possess others.
